To Participate in Athletics Student Athlete's must have a 2.0 GPA and turn in the forms below as well as an emergency card, a physical form a medical doctor, and proof of medical insurance. If you do not have insurance you may purchase school insurance from K & K. The forms are below to enroll on line as well as the optional plans. Please download the forms in either English or Spanish. You will need the consent form, the steroid abuse form, the photo release form, Sudden Cardiac Arrest form, Bully and Hazing form, Liability form
All athletic paperwork is done online at the link below.
Please upload your physical from a Dr. and bring the original to practice.
Please upload a copy of your insurance card.
Please return the final consent form to your coach or Athletic Director.
You may begin to practice once you get an email saying you are cleared to practice.

Please turn in Medical form and the consent form to the coach or Athletic Director.
